Your care journey begins with a personalized one hour visit. During this one-on-one session, our nurses will take time to get to know you, gather important health information, perform an initial assessment, and begin building a foundation of trust. This meeting sets the stage for truly personalized care, with a focus on advocacy, connection, and your overall well-being.

This recurring service is designed to provide consistent, professional oversight, with the ultimate goal of helping you or your loved ones stay safe, healthy, and out of the hospital.

Each weekly visit includes:
✔️ A 1-hour in-home wellness check (90 minutes for two clients)
✔️ Vital sign monitoring: blood pressure, temperature, heart rate, oxygen level
✔️ Assessment of heart, lungs, circulation, and mental status
✔️ Home safety evaluation
✔️ Review and organization of medical records and upcoming appointments
✔️ Medication review, education, scheduling, and pill setup as needed
✔️ Review and explanation of medical test results
✔️ Education on current health conditions and care plans
✔️ Appointment scheduling assistance
✔️ Communication with your medical team when appropriate
✔️ Weekly updates to family members
✔️ Friendly companionship during each visit
